General Information on the Personal Data Law
Within the scope of Article 10 of the KVKK, data owners must be informed before or at the latest during the collection of personal data. Digital Dental Clinic has prepared this information statement in order to fulfill its obligation to inform. This statement includes the purposes for which the data will be processed, to whom and for what purposes it can be transferred, and the rights of the data owner.
In this regard, your Personal Data subject to processing, primarily your personal health data obtained for the execution of all medical diagnosis, examination, treatment and care and similar health services, are listed below:
Your name, surname, date of birth, country of birth, city of birth, gender, marital status, nationality, T.R. identity card information (TCKN, serial number, wallet number, father’s name, mother’s name, place of birth, province, district, neighborhood, volume number, family sequence number, sequence number, household number, page number, registration number, place of issue, reason for issue, date of issue, previous surname), your identity card/driver’s license/passport photocopy,
Your address, telephone number, e-mail address
Your health data, blood group, body measurements (height, weight, head, chest), laboratory and imaging results you submit in your file for tracking, test results, examination data, prescription information, such as health data obtained during the execution of medical diagnosis, treatment and care services your data, consent forms,
Financial data such as your bank account number and IBAN number,
Answers and comments you share for the purpose of evaluating our services,
Camera system image and audio recordings taken during your visit to our hospitals, voice conversation records kept if you contact our call center,
Your private health insurance data and social security institution data for the purpose of financing and planning health services,
Navigation information obtained during the use of our website and mobile application, IP address, browser information and medical documents, surveys, form information and location data you submit with your own consent.

Personal data processed by Digital Dental Clinic is stored only for the period stipulated in the relevant legislation or required for the purpose for which they are processed, in accordance with the law and rules of honesty, accurately and up-to-date when necessary, but only for specific and legitimate purposes. The explanations made for your Personal Data within the scope of this Disclosure Text also cover your Personal Data of a Special Nature.
Persons and Organizations to Which Your Personal Data Can Be Transferred
Within the scope of Transfer of Personal Data, your personal data will be transferred within the framework of the Health Services Fundamental Law No. 3359, the Decree Law No. 663 on the Organization and Duties of the Ministry of Health and its Affiliated Institutions, the Personal Data Protection Law No. 6698, the Private Hospitals Regulation, the Regulation on the Processing and Protection of Privacy of Personal Health Data, the Ministry of Health regulations and other legislation provisions and for the purposes explained above;

In case of private insurance, insurance companies related to provision procedures (health, retirement, life insurance and similar),

The General Directorate of Security and other law enforcement agencies,
